INFORMATION ABOUT USING THIS FORM

Racing members will be issued a timing chip, this incurs an additional cost of $10.00 (refundable at end of season).
Social members do not receive a timing chip but can race untimed if they wish. This would also include family members who do not race.
Guardians are not members but have provided relevant contact details and permissions for a junior membership. Choose this option if you are joining a junior family member but not yourself.
Guardians have to select the family membership option. However, the $55.00 family membership fee will not apply; you will be paying $20.00 for each junior.
If you choose a family membership, or are a guardian, another form will load for details of additional family members; ensure you nominate the correct number of these, you only get one go!
Check the prepaid races box if you wish to save money by paying for your season's racing with your membership. Details on this can be found here.
There is an indemnity waiver at the bottom of this form. You must read and agree to it (by ticking the associated checkbox) to join MTC, or, to be a junior member guardian.

Once this form is submitted (and the subsequent family members form if required) an email will be sent to you to confirm your email address has been correctly supplied. If this email is not received within 24 hours please contact the club registrar. The email will direct you to a webpage for confirmation of your membership application. Once you have visited that webpage a further email will be sent to you with payment options and amount owed. Your membership is not complete until payment is made.

PREPAYMENT OPTIONS:

INDEMNITY WAIVER:

THIS WAIVER COVERS ACCEPTANCE OF MEMBERSHIP CONDITIONS, WAIVER RELEASE/INDEMNIFICATION AND STATEMENTS OF INTENT DECLARATION. WARNING: THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS AND CAN ONLY BE AGREED TO BY PEOPLE WHO ARE 18 OR OLDER. THE CONDITIONS OF THIS WAIVER APPLY TO, AND YOU AGREE ON BEHALF OF, ALL PERSONS INCLUDED AS PART OF A FAMILY MEMBERSHIP.

A contract in the terms set out below between Maitland Triathlon Club Inc and the intending member/s shall be deemed to have commenced on payment to Maitland Triathlon Club Inc and acceptance of a membership application. In consideration of and as a condition of acceptance of my participation in Maitland Triathlon Club activities:

1. I have read, understood and accept that all information contained in this membership form constitutes membership conditions.

2. I hereby attest and verify that I am physically fit and have sufficiently trained to participate in Maitland Triathlon Club activities.

3. I undertake to notify the Maitland Triathlon Club of any change to my medical status or condition, as verified in (1.) above, likely to affect my fitness to compete in the Maitland Triathlon Club activities should this occur following completion of this membership form.

4. I, for myself, my heirs, my executors and administrators hereby waive and release all and any claim, right or cause for action which I or they might otherwise have for, or arising out of loss of life or injury, loss or damage or any description whatsoever, which I may suffer or sustain in the course of or consequent upon my participation in Maitland Triathlon Club activities.

5. I hereby acknowledge that I have sole responsibility for my personal possessions and athletic equipment during Maitland Triathlon Club races and related activities and functions.

6. I hereby consent to receive such medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during Maitland Triathlon Club activities.

7. I agree to comply with all the rules and directions of Maitland Triathlon Club race/event officials and their servants and/ or agents.

8. I hereby allow the free use of my name and picture in broadcasts, telecasts and press of the Maitland Triathlon Club activities as they pertain to various events.

9. I hereby agree that my membership fee shall be non-refundable.

10. I hereby acknowledge this waiver, release and discharge shall be and operate separately in favour of all persons, corporations, associations and bodies involved or otherwise engaged in promoting or staging Maitland Triathlon Club activities and their servants, agents, representatives and officers and includes, but is not limited to, the various committees and members of Maitland Triathlon Club Inc, Triathlon Australia, Triathlon NSW, Maitland City Council, NSW Police, the SES, St Johns Ambulance, CREST, and all other Maitland Triathlon Club sponsors, commercial supporters, suppliers and race volunteers and shall operate whether or not the loss injury or damage is attributed to the act or neglect of any one or more of them.

11. I understand that the Maitland Triathlon utilises a manual timing system for club races and as such, I will be required to have a race number written on my arms and legs and without these numbers I will fail to record any results during the race.
